Track all prompts: embed ID markers in installed files

- Reinstalled all prompts with proper ID marker tracking
- All 5 configured prompts now properly linked to their installations
- No untracked or orphaned installations
- Tracking verified:
  * claude-user → claude:user
  * codex-user → codex:user
  * gemini-user → gemini:user
  * claude-project → claude:project
  * copilot-project → copilot:project
