[BUG] Roo stuck in endless loop, not responding to any instructions

Roo stuck in endless loop not responding to any instructions

Context (who is affected and when)

It acctually happend 2 times. Roo is stuck in fail loop repeating the same thing over and over again and not listening to any instructions. Changing AI models didnt help , chaning mode from code to ask also didnt help (ask just printed command same as code but as ask mode cannot use commands command wasnt used). Only starting a new task can help where it starts fresh. So it isnt a LLM problem and it isnt a prompt problem either. It just roo stuck in the loop and maybe sending same API request all the time idk.

Reproduction steps

1. WIndows 10
2. Big promt for orchestrator mode then he created new tasks for other modes
3. I think it happens after context shrinkining and then roo just dont follow any instuction before context shrinkining roo followed instructions and for example tried different approach

1. Logs: Could you please share the logs from the Roo Code Output panel? (View > Output > select "Roo Code" from the dropdown). The logs from when the loop started would be especially helpful.

2. API Provider and Model: You mentioned "None" for API provider and did not specify a model. Which API provider (e.g., Anthropic, OpenAI, Bedrock, etc.) and specific model were you using when this happened?

3. What is repeating?: When you say Roo is "repeating the same thing," could you describe more specifically what is being repeated?

   • Is it the same tool call (like read_file or execute_command) with the same parameters?
   • Is it the same text response/message?
   • Is it attempting the same action that fails and then retrying?

4. Auto-Approval Settings: Do you have auto-approve enabled for any actions? This could affect how the retry loop behaves.

5. Reproducibility: You mentioned this happened twice. Do you have a way to consistently reproduce this issue? If so, could you share the steps?

6. User Messages: When you send new instructions during the loop, do they appear in the chat UI? Does Roo acknowledge receiving them at all, or does it completely ignore them?

This is the tool call that Ask mode typed and is reapeated by all the modes. In first time it was read file.

I've looked at your logs and can see the issue clearly now. The key observations are:

1. Repeated tool call: The execute_command tool with npx jest --config tests/config/jest.config.js --listTests is being called repeatedly
2. Command fails each time: The error shows "Can't find a root directory while resolving a config file path"
3. User messages ignored: Your instructions appear in the UI but Roo doesn't respond to them
4. Auto-approval setting doesn't help: This happens with or without auto-approval enabled

Based on the logs and code analysis, this appears to be a case where:

• The model keeps retrying the same failing command
• The existing ToolRepetitionDetector should catch this, but there may be subtle differences between retries that cause the detector to not recognize them as identical
• User interruptions are not being properly processed during the active streaming/tool-execution state

I'm investigating the tool repetition detection logic and how user messages are queued during tool execution. Will open a PR with my findings and potential fix shortly.

@Dakarr1 I've opened a draft PR with my attempt at addressing this issue: #10816

The fix adds queue message processing at two critical points:

1. In Task.ts - When consecutiveNoToolUseCount >= 2 (model not using tools), we now check for queued user messages and inject them as feedback
2. In presentAssistantMessage.ts - When ToolRepetitionDetector detects identical consecutive tool calls, we check for queued messages before asking the user

This should allow user messages sent during auto-approve loops to break through and redirect the model.

Problem (one or two sentences)

Roo stuck in endless loop not responding to any instructions

Context (who is affected and when)

It acctually happend 2 times. Roo is stuck in fail loop repeating the same thing over and over again and not listening to any instructions. Changing AI models didnt help , chaning mode from code to ask also didnt help (ask just printed command same as code but as ask mode cannot use commands command wasnt used). Only starting a new task can help where it starts fresh. So it isnt a LLM problem and it isnt a prompt problem either. It just roo stuck in the loop and maybe sending same API request all the time idk.

1. WIndows 10
2. Big promt for orchestrator mode then he created new tasks for other modes
3. I think it happens after context shrinkining and then roo just dont follow any instuction before context shrinkining roo followed instructions and for example tried different approach
