Use form-data for multipart upload #450

Use https://github.com/form-data/ for multipart upload rather than do it by hand in order to get correct content disposition. @jhiesey mentioned seeing some odd behavior here from the SDK. form-data gets it right and is battle tested. Lets just use it.
