Enable Codeowners across the repo

Setup Code Owners, and require code owner approval via branch protection rules.

Note: I accidentally force-merged the initial codeowners file to main, so there's no PR for it. I was doing this as part of testing branch protection rules, and didn't expect the push to go through.
