Smarter hover actions for Exception subclasses when debugging

Quite often when I break on an exception I'll have to ⬇ in the debugger. It would be neat if I could see that info when hovering e, optionally with a clickable link to dump to places in the stack.

The hover content when debugging comes from debugpy, moving this there to look into first. Likely, we might need to move this to core to allow action contributions from debugger.
