I think some where profile update got broken. Can you check if the profiles for each of these cases was updated, and when you re-trigger shell start up is there any error reported.

No errors reported. The profiles were updated.

Yeap, this shouldve been fixed.

Only time command will run when shellStartup is set, is when user is prompted but do not click allow profile script modification.
Then we send command to still ensure activation.
