What is the change?
Use OIDC sub as the main unique identifier and enforce presence of email and NHS Number
Why are we making this change?
To ensure that OIDC claim is entirely unique and stays that way and that required fields are enforced at the db level
Notes
Some changes to user are unable to be backfilled. Migrations have been squashed to one initial migration and environments can be reset now before we have any meaningful data in prod. This will require re bootstrapping our database on first deploy
