- Introduce dify provider in provider.ts with custom model
- Add dify-headers.ts for dynamic header application:
- Extract conversation ID from message metadata
- Detect compaction/summary contexts to preserve or reset chat continuity
- Handle synthetic user messages and real requests appropriately
