This PR changes migrations naming scheme so that migrations are grouped by year and ordered by date to make it easier to locate and work with migrations when there are many of them. (We already have 93 migrations.) Existing migrations are renamed to match the new scheme. Revisions stay the same, so there is no effect for existing databases.
