- pull the latest version from https://sqlite.org/src/file/ext/misc/tmstmpvfs.c
- fflush after every write to avoid buffering
- fix salt1 offset in the WAL frame header (it's at offset 16 in the WAL Header, but offset 8 in the WAL Frame Header)
