CCM-12949 Add campaign ID to choose templates page

Description

• Adds campaign ID onto the choose templates page
• Updates the copy used on the "Change name" link to "Edit settings"

Context

As an integrator team member
I want to view the campaign id associated with my message plan on the choose templates screen
So that I can confirm which campaign the message plan is intended for

Type of changes

• Refactoring (non-breaking change)
• New feature (non-breaking change which adds functionality)
• Breaking change (fix or feature that would change existing functionality)
• Bug fix (non-breaking change which fixes an issue)
